The team works together to provide a psychiatric review, prescribe medication and provide an assessment for your social care needs. We conduct an initial assessment and create a recovery plan with you, this will map out how long we anticipate you will require our support. After three months it will be reviewed and we work out whether you require a long term intervention, if so we will place you on the care programme approach.
