We provide high quality mental health care to people with psychotic illnesses and our goal is to support recovery, to promote mental and physical wellbeing and optimise independence and social inclusion.
The team works together to provide a psychiatric review, prescribe medication and provide an assessment for your social care needs. We also monitor your physical health, have walking groups which you might like to join, provide smoking cessation support and are able to link in with other agencies if necessary.
For those diagnosed with with a psychotic disorder including bipolar.
We conduct an initial assessment and create a recovery plan with you, this will map out how long we anticipate you will require our support. After three months it will be reviewed and we work out whether you require a long term intervention, if so we will place you on the care programme approach.
We are developing a stronger focus on wellbeing and physical health
Emphasis on identifying and supporting service users with their social care needs
Strong links to other agencies
