The Intensive Support and Re-ablement Team

The Intensive Support Team (IST) provide up to two years with clients on a team approach basis. The Re-ablement Team offer short term intervention upto 6 weeks with allocated key worker. We provide support to improve people's independence and to resolve some of their practical issues if possible. We prioritise referrals for those who are most in need and provide interventions to support / facilitate hospital discharge and to avoid hospital admission or to prevent relapse. The team works within the principles of the recovery model and focuses on helping to rebuild services users’ confidence and skills around practical day to day tasks by doing things with rather than for them.
