Camden Minding the Gap Transitions Team

We are a service for professionals that facilitates the transition of young people from agencies working with young people, (including Child and Adolescent Mental Health Services) into adult service, and also reviews cases in adult services where there is concern about disengagement and risk. The service is for Camden residents only.

Senior clinicians from adult and young people services assist referrers in developing a management plan for young people. Fortnightly case discussion meetings are open to professionals from all agencies. In addition, clinicians are based within specific teams to champion the cause of young people and help develop a more young-people friendly environment. They assist both with transition from CAMHS and work with young adults coming into services for the first time.

The fortnightly case discussion forum for professionals takes place at St Pancras Hospital. Access to Transition Champions will be via this forum or via the specific teams.


This is a service for agencies working with young people that are seeking advice and help with the transfer and management of people who are moving from CAMHS into AMHS settings; and professionals who are concerned about service users disengaging and/or risky behaviours in the context of their mental health needs.

You will receive advice and, where agreed, fortnightly case discussion along with additional support provided by staff in identified teams.
  • Transition Champions are based within key services to provide additional support 
  • The fortnightly case discussion forum brings together key senior professionals from CAMHS / AMHS to ensure that prompt and effective decision-making for complex cases can take place
  • There is a ‘no bounce’ and ‘no delay’ principle in place ensuring that service user’s needs are effectively assessed, prioritised and managed across agencies.
