The 18-24 Transitions Team aim to understand young people's lives from their point of view in order to enhance their potential for change. The team offer mental health, psychology and substance use assessments, using results to develop plans with the young people for intervention or onward referral.
We work to reduce gang related offending and serious violence and harm to young people, their families and local communities. In addition we increase access to opportunity by helping young people to gain employment, education and training by working closely with statutory and non-statutory agencies to provide comprehensive case management and prevent duplication of work.

The 18-24 Transitions service provides intensive support to 18-24 year olds in London (Islington) who are at greatest risk of involvement in serious youth violence. This is a group of young people with complex needs who often do not access the psychological or mental health support they need.
