Routine referrals

There are three main routes to referral 

  1. Direct referral by a health professional
  2. Referral through our Assessment and Advice team
  3. Self referral

Direct referrals can be made by GPs or any other health, social care or emergency services professional to most of our services. But not all services can be accessed this way; Community Mental Health, psychosis, Outreach and Services for Ageing and Mental Health you will need to contact the Assessment and Advice Team.

Our community teams offer focused support to people being treated in the community. Separate teams support those with psychotic disorders such as schizophrenia and bipolar disorder (Recovery and Rehabilitation, Outreach Services) and non-psychotic disorders such as depression, trauma and personality disorder (Community Mental Health Services).
