What is DIALOG+?
DIALOG: is an 11-question patient led outcomes scale whereby people with a mental health illness are asked to rate their satisfaction and needs for care on 8 life domains and 3 treatment aspects including:
- Your mental health
- Your physical health
- Your job situation
- Your accommodation
- Your leisure activities
- Your partner/family
- Your friendships
- Your personal safety
- Your medication
- The practical help you receive
- Consultations with mental health professionals
DIALOG+ is a facilitated conversation focusing on the lowest scored areas using the DIALOG scale. The Healthcare Professional and Service User work in a strength-based therapeutic way, co-producing a holistic care plan.
Why use DIALOG+?
• It is very simple to use, and only has 11 questions. It enables proactive, personalised conversations at an individual level, supporting self-management and helping service users move forward with their journey of recovery.
• It has been used to help inform the redesign of care planning processes within mental health services.
• Capturing this information gives a powerful indicator of patient satisfaction levels where health and social care services need to focus for improvement.
• It is designed to measure how service user rate quality of life and experiences of the care received.
• Responses can help mental health professionals to make sure we talk about what matters to the individual.
As a minimum, it is recommended that DIALOG+ is completed at the start of an episode of treatment, during review dates and routine appointments and at the end of an episode.
