The Intensive Support and Re-ablement Team

We provide intensive support over a six-eight week period to improve peoples’ independence and to them to resolve some of their practical issues if possible. We prioritise referrals for those who are most in need and provide interventions to support / facilitate hospital discharge and to avoid hospital admission or to prevent relapse. The team works within the principles of the recovery model and focuses on helping to rebuild services users’ confidence and skills around practical day to day tasks by doing things with rather than for them.

We help adults with mental health problems 

You will be allocated a keyworker to meet with regularly - this is usually two to three times a week for six to eight weeks. We will help you with your support needs and your mutually agreed goals. We can support you in areas such as: daily activities such as shopping, cleaning and cooking (prompting); benefits and housing issues; reducing isolation; emotional support; accessing local community services, leisure activities and education; budgeting; managing medication (prompting); attending appointments; and registering with a GP.
